This authorization is to remain in full force and effect until M圜ommerce has received notification from me of its termination in such time and in such manner as to afford M圜ommerce, its agents, and JPMorgan, a reasonable opportunity to act on it. I further represent that I am at least 18 years old and acknowledge and understand that the origination of transactions to my payment account must comply with the applicable provisions of United States law.
I acknowledge and agree that usage of this service constitutes my acceptance of the Electronic Payment Terms and Conditions for payment. I hereby authorize M圜ommerce and JPMorgan to make debit and credit entries to my account, at the depository financial institution indicated by the routing transit number listed. M圜ommerce will electronically pass the required encrypted data to JPMorgan, NA to facilitate my transfer of funds.
